About Terra

We are building the workspace we wanted clinicians to have.

Clinicians spend hours each day on documentation, much of it after the last patient leaves. Terra exists so the note is written while the visit happens, from sources the clinician can check, and signed before they go home.

Why Terra

Documentation should follow the visit, not the other way round.

Most AI scribes produce a block of text at the end of a visit and leave the clinician to work out where each sentence came from. That shifts the work rather than removing it: the note still has to be checked against the conversation and the chart, line by line.

Terra starts from the opposite end. Every sentence in a draft carries a citation to the transcript segment or chart source it relies on. The chart's context (approved allergies, active medications, what changed since the last signed note) is gathered before the visit and labelled as history. Code suggestions arrive with the exact excerpt that supports them. When something is not documented, Terra says so.

We build for independent clinicians and small groups first, because they do their own documentation and feel every extra minute. Terra runs in the browser, needs no EHR integration to start, and is priced so a solo clinician can compute what a month costs.

Principles

How we decide what Terra does.

  • Drafts, never decisions

    Terra proposes. A clinician reviews, edits and signs. Nothing enters the record on its own, and a signed note is frozen with a hash.

  • Every claim has a source

    Sentences cite the transcript or chart. Codes carry the excerpt that supports them. Answers cite the note, document or study they rest on.

  • Minimum necessary, by default

    Prompts carry age, pronouns and visit details, never names or MRNs. Evidence searches are de-identified and confirmed before they leave Terra.

  • Say what is missing

    An unsupported section reads [Not documented]. A question the chart cannot answer gets “not found”, not a plausible guess.

  • Prices you can compute

    Plans are priced in credits at published model rates, with an estimate before every paid operation and unused credits returned.

  • Built with clinicians

    We design each workflow with the people who chart at night, and we change it when they tell us it is wrong.
