The chart, already read

Context Engine

A draft that already knows the chart.

Terra's Context Engine writes the visit context before you start: age, approved allergies, active medications and what changed since the last signed note.

  • What it does
  • Pulls clinician-approved allergies and active medications
  • Summarises what changed since the last signed visit
  • Every context line shows its source; you pick what the draft may use

Context Engine, on a synthetic visit

A synthetic visit, played in the layout of the app. It starts when it scrolls into view and loops.

Animated demo: the Context tab writes itself from the chart: age, allergies, active medications and what changed since the last signed note, each with its source.

How it works

Context Engine in three steps

  1. 1

    Open the visit

The Context tab fills in from the chart: demographics for the date of service, approved allergies and active medications.

  2. 2

    Choose the sources

Toggle prior signed notes, problems and documents in the source picker. Only what you enable reaches the draft.

  3. 3

    Draft with context

The note draft cites chart sources separately from the transcript, so you can tell what was said today from what was already known.

In detail

What it actually does

The specifics, written from how the product behaves today.

  • 01

    Approved facts only

Allergies and active medications come from clinician-approved entries. Pending, retracted or changed-source facts are excluded from the draft.

  • 02

    You choose how far back

Include up to three prior signed notes and the active problem list per visit. The choice is saved on the visit, so the next draft uses the same sources.

  • 03

Labelled as the chart, not today

Chart context is cited separately from the conversation, so a reviewer can tell what was said in the room from what was already on file.

  • 04

    No names in the prompt

The encounter header uses age at date of service, pronouns and visit details. The patient's name and MRN are never sent to the model.
