How it works

    Sign or save the note

    Open the coding worksheet from a saved visit. Terra shows the credit estimate before any suggestion runs.

    Review suggestions

    Each candidate code arrives with the exact note excerpt behind it. Unspecified codes come with a prompt for the missing detail.

    Confirm

    Accept, change or reject each code. Terra checks the excerpt still matches the saved record; nothing is submitted for you.

What it actually does

    The excerpt has to be in the note

    Every suggested code carries an exact excerpt that must appear word for word in the saved note. A suggestion whose excerpt does not match is rejected.

    Checked against the official release

    Codes are validated against the ICD-10-CM release for the date of service. Category headers and codes missing from the release are replaced with the nearest valid codes, and the replacement is labelled.

    Documentation state, not just a code

    Each candidate is marked confirmed, possible, history or ruled out, so a ruled-out diagnosis is never presented as a confirmed one.

  • 04

    Estimate first, then run

    You see the credit estimate and the number of sources before suggestions run. Only the saved note is sent; the transcript is included only if you choose it.
