    Prepare

    Assistants set up visits, upload documents and review transcripts. Roles decide who can see and change what.

    Sign

    The clinician signs. The signed text is frozen with a SHA-256 hash; later changes become attributed amendments.

    Co-sign

    Request a co-signature from another clinician. They review a read-only snapshot and attest or decline with a reason.

    Sites, care teams and restricted charts

    Organize clinicians into sites and care teams, and restrict a patient's chart to selected people or teams. Restricted charts are left out of search, review queues and the library for everyone else.

    Signatures that do not move

    A signed note is frozen with a SHA-256 hash. Every later change is an attributed amendment, and the original signed text stays readable.

  • 03

    Independent co-signatures

    A second clinician reviews a read-only snapshot of the signed note and its amendments, then attests or declines with a reason. If the record changes first, the request goes stale.

  • 04

    One review queue

    Reviews collects work needing attention, work assigned to you and work you requested, so handoffs do not get lost in messages.
