How it works

Start the visit
Press Capture on a visit. Terra records in the browser; no extra hardware or app to install.

Get a cited draft
When you stop, the note is drafted section by section in your template, with [S1]-style citations to what was said.

Review and sign
Edit inline, accept or reject sections, and sign. The signed text is frozen with a hash so later edits become amendments.

Drafts only from sources you can see
Today's transcript, template fields and the documents you selected come first; chart context follows as separately labelled sources. Before a draft runs, Terra lists every source it will use.

[Not documented] instead of a guess
A section with nothing to support it reads [Not documented]. Terra flags it in the draft rather than filling the gap with plausible text.

History stays history
Prior notes are attributed (“Per prior note dated …”) and never presented as today's exam, results or plan. A missing allergy entry is never written as “no known allergies”.

Stale drafts are caught
Every chart source is pinned by version and hash. If an allergy is approved or a prior note is amended while the draft is being written, the draft is marked stale before you see it.
