Answers come from approved facts, reviewed documents and signed notes. Unreviewed extraction output and pending facts are excluded.

A citation on every claim
Each statement carries an [S1]-style chip that opens the exact note, document excerpt or transcript segment it came from.

Says so when it cannot find it
If the record does not contain the answer, Terra says it could not find it, and reminds you that no match is not evidence that something did not happen.

Scoped to the patient in front of you
Questions stay within the current patient and visit. Searching across patients requires an explicit organization-wide scope, and restricted charts are never included.
