Ask questions about a patient's record and get answers that cite the exact excerpts they came from.

Record chat answers questions from the patient's record, with a citation for every statement. When the record does not contain the answer, it says so.

Ask a question

  1. Open a visit or a patient.
  2. In the AI chat column on the right, type into Ask about this patient's record…, or pick a suggestion such as "Summarize this patient's record" or "What allergies and active medications are documented?".
  3. Send the message.

Each answer cites its sources. Select a citation to open the exact excerpt in the visit, document, fact or signed note it came from.

What chat can read

  • This visit's context and transcript.
  • Documents with approved, reviewed text.
  • Approved facts: allergies, medications and problems.
  • Verified signed notes, with their amendments.

Unreviewed extraction output, other patients and unsigned drafts from other visits are not included.

When the record does not say

If nothing in the record supports an answer, Terra replies that it is not in the record rather than filling the gap. Absence from the record is not the same as absence in the patient, so check before relying on it.

Literal excerpts

When no AI provider is connected, or when you switch to literal mode, chat returns the exact matching excerpts from the record without any AI interpretation. This uses no credits.

For a straightforward text search across one patient, or across every chart you can access, use Search reviewed records from the patient page or the Library.

Credits

Answers use credits based on the length of the question and the record sections read. Clear conversation starts a new thread.

Good questions to ask

  • "When was metformin started, and at what dose?"
  • "What did the last two signed notes say about the knee?"
  • "Which documents mention a penicillin reaction?"
  • "What changed in the medication list since the last visit?"

Chat answers what the record says. It does not interpret results, diagnose or recommend treatment. Verify every answer before using it in care.
