Imports and busy front desks sometimes create two charts for one person. Terra finds likely pairs and lets you record a decision. It never merges charts.
How pairs are found
Two records are shown as a possible duplicate when either:
- both have the same MRN (ignoring outer spaces and letter case), or
- both have the same full name (ignoring letter case and extra spaces) and the same date of birth.
There is no fuzzy or AI matching. Matching fields alone do not prove that two records are the same person.
Review a pair
- Open Patients and choose the duplicate review.
- Pick a pair. Terra shows the original name, date of birth, MRN and chart ID of both, and which rule matched.
- Choose an outcome:
- Investigate: more checking is needed. The pair stays open.
- Distinct people: the records stay separate.
- Same person: you have confirmed both records describe one person.
- Enter a reason and confirm.
Only a clinician who manages the practice can record an outcome. Assistants and clinicians can view pairs for patients they can access.
What a decision changes
Nothing in either chart. Visits, signed notes, documents, facts and access stay exactly as they were. "Same person" records the investigation outcome so the team knows which chart to use going forward.
Reopening
A decision can be reopened with a reason. If either profile changes later, even a pronoun, the earlier decision becomes stale and the pair returns to the open queue for a fresh look.
Privacy
You only see pairs where you can read both records. If you lose access to either one, the pair disappears from your list.
