Each patient has one profile that collects their visits, documents, reviewed facts and follow-up tasks.

Create a patient

  1. Open Patients in the left rail and choose New patient, or create the patient while starting a visit.
  2. Enter the name. Add date of birth, MRN and pronouns now or later.
  3. Save.

An MRN must be unique within the practice. Age in drafts is calculated from the date of birth at the date of service, so keep it accurate.

The tabs

  • Overview: the prepared record brief and what changed since the last signed note.
  • Visits: every visit for this patient, newest first, with its status.
  • Documents: uploaded letters, results and outside records, with their review status.
  • Facts: reviewed allergies, medications and problems. See Facts and documents.
  • Tasks: follow-ups with an owner and due date.
  • History: a timeline of visits, signatures, amendments, document reviews, fact changes and task events.
  • Settings: site and access restrictions, for clinicians who manage the practice. See Sites, care teams and patient access.

Follow-up tasks

From Tasks, choose New follow-up task. Give it a title, optional details and due date, and assign it to a clinician or assistant who can see this patient. Tasks move between Open, In progress, Completed and Cancelled. Each change needs a short reason and is kept in the task history. A task is overdue when it is still open and its due date has passed (dates are UTC days).

Finding a patient

Use the header search (⌘ K on a Mac, Ctrl K on Windows) to search patients, visits and templates. The Patients list can also be filtered by site and care team.
