Nothing in Terra is final until a clinician signs it. This is the path from draft to a record you can copy into your EHR.
Review the draft
- Open the Note tab.
- Read each section. Select a citation to see the transcript turn, context or chart source it came from.
- Edit freely. Your edits are saved against the visit version.
- Replace every
[Not documented]with your own text, or leave it if nothing was documented.
Check readiness
Visit workflow → Documentation readiness compares the saved note with its template. It lists missing required sections, unfilled placeholders, document sources that changed, unfinished jobs and open review requests. Unsaved edits are labelled and do not count. Readiness checks documentation, not clinical correctness.
Sign
- Choose Mark reviewed, then Sign note.
- Terra repeats the readiness checks at the moment of signing.
- The signed text is frozen with a SHA-256 hash and your name, credentials and time.
In practices with more than one clinician, you can ask a colleague for a co-signature from the signed visit's Co-signature panel. The co-signer attests to a frozen copy; it never replaces your signature.
Add an amendment
A signed note cannot be edited. To correct or add information:
- Open the signed visit.
- Choose Add amendment, write the change and save.
The amendment is dated and attributed and appears under the original text. Any approved coding worksheet becomes stale and needs a fresh approval, because the record it was based on changed.
Export
- Export note produces the signed text with its amendments and any completed co-signatures, ready to paste into your EHR.
- Visit workflow → Export package saves an immutable package. When you download it, Terra checks the file against its SHA-256 before saving it to your computer.
Terra does not send notes to an EHR or submit claims.
