Custom templates let you document the way you already do. Terra keeps every version, so changing a template never rewrites a note that used an earlier one.
Create a template
- Open Templates in the left rail and choose New template, or open an existing template and duplicate it.
- Name it and add sections. For each section, write a short instruction for what belongs there, for example "Medications changed today, with dose and reason".
- Mark sections required or optional.
- Save. Each save is a new, immutable version.
Import from a sample note
You can start from a note you already like:
- Choose import, then paste text or upload a Word document, Markdown, a text-based PDF, or a scanned page or photo.
- Terra proposes the headings it finds. The narrative under them is left out.
- Review the proposed sections, rename, reorder or exclude them, and mark required ones.
- Save as a draft.
If headings contain names or other identifiers, Terra asks you to remove them before saving.
Typed fields
Sections can contain fields with a type, such as a choice, yes or no, a number or a date. Fields have no default answers. A section or field can be shown only when an earlier choice has a particular answer, for example a smoking-history section that appears only when "Current smoker" is Yes.
Publish
A template is a draft until it is published.
- Open the saved version you want to use.
- Choose publish, add a short reason and confirm.
Personal templates are visible only to you. Practice templates need management permission to publish or change, and are visible to the whole practice once published. While you work on a replacement draft, the current published version stays selectable.
Archive
Archiving removes a template from new visits. Visits that already use it keep their pinned version, and signed notes are never affected.
